 Your safety is our primary concern. No one may participate in any hunting or shooting activities unless this Waiver Form is completed. You must follow safe gun handling and hunting procedures. No alcohol or drug consumption is allowed before or during the hunt. You are responsible to make sure that you have the proper safety hunting course required by law and hunting permits. If anyone is found to be breaking the law or safety violations, they will be asked to leave immediately with no refunds. You are responsible for all safety and hearing protection as needed 

By my signature below, I certify that I am fully aware of the hazards involved in the sport of hunting and understand the necessary actions to assure hunter safety. I am also aware of the hazards associated with a Waiver Form and/or ranching operation.

In consideration of being permitted to participate in the sport of hunting /fishing conducted by Cody Mays Outdoors Hunting & Fishing Charters, I am willing to assume these hazards and do recognize they can be dangerous to both life and limb.

In full recognition of such, I hereby release, waive and discharge Cody Mays Outdoors, its owners, employees, or people with whom they contract for services, including all landlords, ( Releases), from all liability to the undersigned, my spouse, legal representative, heirs, and assigns, for any and all loss, claim, or damages resulting there from, on account of injury to me, children, spouse, or my property, even injury resulting in death, while participating in the sport of hunting, shooting and fishing.

This release and wavier is complete as it is my intent to hold harmless and indemnify Releases for any injury that might result while I am involved in the sport of hunting, shooting and fishing conducted by Cody Mays Outdoors while on any of its premises whether it be by land or water.

The undersigned assumes full responsibility for the risk of bodily injury, death or property damage due to the actions or inactions of Releases. The undersigned agrees this release, wavier and indemnification agreement is intended to be as broad and inclusive as permitted by the laws of the State of Texas.
